based on the IP and properties (DNS name, port #)

Custom devices enable you to monitor technologies where Dynatrace OneAgent can not be installed like Firewalls, DataPower gateways, cloud databases, or any other network appliance such as a proxy or gateways.

for example: if you see some transactions are going to unmonitored host and you can't install oneagent, you create custom device

Long story short
Based on mapping 3-rd party host/IP,
Dynatrace creates a virtual object which mimics some capabilities of monitored service.

You literally say: Dynatrace please name all requests from this host as "My Star Service"
But this is just a nice grouping request from 3-rd party services. It helps well to monitor 3-rd party dependencies.
