how can we create alerts for mssql database status monitoring (extension) ?

unable to find any metric related to the same in custom metric alerts configuration

You can try to pick any metric available and check the Alert on missing data when creating the metric event.

If the DB connection fails, the metric will stop to receive data and you will get a problem for it.
