more than 25 failures in a period.

As far as I know there is nothing like this which is built-in. There are some alerts that trace IP addresses like HotIp or TravelUser but they do not collect information about failures.

What could be done in theory is a creation of new sql alert when you can define an sql query to fetch the data you need. I would strongly discourage that, though. This could be a performance-killer and slow down the whole CAS. Plus it would be only working on a single node environment.

In 2018 we introduce the ability to use user name dimension in alerts so it would be sufficient for the mentioned purpose.
