Could you please mention the approach for setting up failure alerts in case of a submission form failure?

Dynatrace automatically notifies you if the error rate increase on that form shows high statistical evidence. If you would rather alert on single request failures you have to set up a static threshold on that key service request which you can find within Service Screen - Edit - Anomaly detection - Key requests.
