The user might have set the same DNS name for multiple nodes. Further symptoms which can be observed are, that upgrades will fail and when collecting a support archive, it will contain logs only from one node.

In CMC it can be seen that the installation can't be synced:

As a solution user needs to correct the DNS or file /etc/hosts.
